Behavioral Health Counselor at Family Health Center in Holly Hill, SC
DESCRIPTIONJOB FUNCTIONS:
Serve as a patient advocate within the FHC system. Receive and review referrals for behavioral health and social work. Provide direct services to patients and families through case work and/or group work. Assess, plan and implement treatment plans, including providing counseling, psychotherapy, and follow up with appropriate intervention for each referred patient and modify treatment plan in accordance with patient progress. Evaluate clinical symptoms, developmental and personality disorders, psychosocial stressors and global functioning. Assist patients and their families in understanding and conforming to prescribed treatment(s). Link patients and their family members to resources that provide needed services. Establish and maintain accurate and complete case records, forwarding notes to healthcare providers as appropriate. Maintain relationships with community social service agencies and collaborate in developing services to meet the health related needs of the community. Exhibit compliance with FHC's corporate compliance policy. Performs job activities in ways that are respectful, courteous and responsive while interacting with patients, visitors and co-workers. Perform other duties and assume other responsibilities as appropriate or as directed. PHYSICAL REQUIREMENTS:
Must possess ability to communicate in the dominant language of the geographic region. Must be able to lift 30 pounds. Vision and hearing corrected to within normal limits is required. Must be independently mobile. POSITION REQUIREMENTSA master's degree in social work from an accredited college or university supplemented by five years experience as a clinical social worker with two years of experience performing case management is required. Must possess a license to practice as a Licensed Master Social Worker (LMSW) (LPC, LISW, LISWCP accepted) in the State of South Carolina. Must have a valid South Carolina driver's license.
Salary Range:
$80K -- $100K
